Definition
Relating to motor control pathways in the brain outside the pyramidal tract, particularly involving the basal ganglia and associated structures. Extrapyramidal symptoms include movement disorders like tremor, rigidity, and abnormal postures, often caused by medications or neurological diseases.
Etymology
From Latin 'extra-' (outside) and 'pyramidalis' (pyramid-shaped), referring to motor pathways outside the pyramidal tract. The term emerged in early 20th-century neurology as understanding of brain motor systems advanced and different movement control circuits were identified.

The extrapyramidal system is like the brain's 'movement editor' - while the pyramidal system sends direct movement commands, the extrapyramidal system fine-tunes movements, controls posture, and enables smooth, coordinated motion. When antipsychotic drugs block dopamine in this system, patients develop the characteristic stiffness, tremors, and abnormal movements called extrapyramidal symptoms.
